Capital Punishment

Somehow, while looking up vids on youtube, I AGAIN became curious about Capital Punishment (which Michigan does not have) and thought of the old question "what was the best way to execute people?". I believe I talked about the electric chair before and I read about its failures.
So, my next thought was "being beheaded". Decapitation!
This comes to the age-old Guillotine, cutting the head by using a sharp blade with gravity. I naively thought that it would be a perfect C.P device, but each device should have its mistakes. What could be the guillotine's? I don't mean like "it shouldn't be used because we see it".
I'm thinking how it can fail in killing the convict quickly. What could it be? I never looked it up yet.
What if it failed to cleave through the neck for example (get stuck)? Or maybe you can stretch out your neck in between the locks and that makes the blade miss were it should cut?
Something like that.
